- Registered Nurse with Singapore Nursing Board
- Minimum 3 years’ experience working in Restructured Hospital settings
- Home nursing experience and Advance Diploma in relevant nursing specialties preferred
- Experience in Palliative Care, Pediatric Care, OR ICU setting preferred.
- Main Responsibilities & Tasks
- Provide nursing care in the home environment e.g. ADL assistance, wound care, nasogastric tube management, patient/caregiver education; ensuring at all times:
- Dignity of the patients
- Patient’s safety
- Confidentiality of the patients’ information
- Advocacy on behalf of the patients
- Provide coordination and care management for assigned patients by
- Assessment & Care Planning
- Implementation of care & Outcome of evaluation
- Make referral to other relevant healthcare providers as needs arise
- Provide nursing care in the home environment e.g. ADL assistance, wound care, nasogastric tube management, patient/caregiver education; ensuring at all times:
- Ensure timely submission and accurate documentation of patient’s condition and care given
- Feedback nursing report to referring partners as required.
- Review and discuss with Nursing Manager or COO on any problems encountered in patient’s condition and/or in the handling of patients and/or care-givers.
- Provides teaching and counsels to both patients and families and evaluate the teaching programmes to meet patients and caregivers needs and expectations.
- Orientate and coach new nurses and serve as liaison, resource person and preceptor to nurses and trainees, including new JagaPros
- Maintains a continuing knowledge of competencies related to the Nursing profession by participating in formal education programs, conference, workshops and professional organisations.
- Other duties and tasks as assigned.
Full-time position. Monday-Friday with rotating evening and weekend on-call shifts.
